1. Although primary hypertension, which is high blood pressure due to unknown causes, accounts for 95% of hypertension cases, high blood pressure can also occur due to other disorders. It is then termed secondary hypertension. Can you identify 23 causes of secondary hypertension?
2. Reflect on a patient you have recently cared for with hypertension. How was the diagnosis arrived at? For example, was it based on several clinic BP readings, ABPM or HBPM? Was it taken in both arms?
3. NICE guidelines recommend that the diagnosis of hypertension should be based on the clinic measurements as well as ABPM. Indeed, the ESH/ESC (2013) report also recommended that the role of ABPM and HBPM should beexpanded, as they are better predictors of outcomes. However, if your patient was unable to tolerate ABPM, refused it or ABPM was unavailable, can you suggest another recommended adjunct to clinic blood pressure to confirm
the diagnosis?
4. Automated BP devices are often used to assess blood pressure and pulse levels. If your clinic did not have access to a manual device, how would you ensure the reading was accurate?
5. Before reviewing the next section, use the information about the physiology of blood pressure control to briefly outline how key drugs, such as ACE-inhibitors, beta- blockers and diuretics, reduce blood pressure. You may wish to consult a physiology text, such as Tortora & Derrickson (2010), to review the physiology of blood pressure control.
