Sally is a 21-year-old woman who came to see you after having an epileptic seizure and was unable to wake up. Sally has had other seizures in the last two years, according to her father, all of which have been
linked to her sleep pattern. Due to the spasmodic nature of her seizures and her difficulty in obtaining a witness account, she has never been medicated before. Sally was referred for an electroencephalogram
(EEG) with sedation after a consultation. She had another seizure during the appointment, which was witnessed. Sally had hemiparesis after the seizure, with drooping eyelids and a drooping mouth, as well as
being incontinent of urine. Sally was admitted, and an EEG revealed that she had temporal lobe epilepsy with tonic-clonic seizures (complex). She was rushed to the hospital for an emergency scan, which
revealed scar tissue in her left temporal lobe, which was thought to be the result of a childhood accident. There are no tumor signs, no fever, and all biochemistry results are normal. The hemiparesis went away
after an hour. Sally’s boyfriend was concerned that she had a brain tumor or had had a stroke, but Sally’s doctors confirmed that she had a residual Todd’s Palsy that had resolved after she had recovered from
her seizures.
