Describe a scenario where you are participating in an interprofessional team that is performing a root cause analysis on a failure-to-rescue or FMEA scenario when the chain-of-command was not utilized.
Describe a scenario where you are part of an interprofessional team that is trying to improve processes related to one CMS core measure, such as for heart failure.
Identify a real or virtual patient care error, perform an analysis of the causes of the error, and identify quality improvement strategies to address the identified cause(s).
