Lavinia Adjei is 50 years old. She has suffered from hypertension and type 2 diabetes for the past four years. For these long-term conditions, she takes aspirin 75mg daily, enalapril 20mg daily, simvastatin 20mg every night and
metformin 500mg three times a day. She was also diagnosed with Crohns disease when she was 20 years old. This illness is usually well managed by diet alone and she has been in remission for the last 10 years. Investigations
at that time confirmed distal iliac disease. Lavinia goes to the gym on a regular basis, does not drink alcohol but does smoke about between 5 and 10 cigarettes per day, depending on her level of anxiety.
Due to her recent promotion to a senior position in her company and the increased level of stress, her smoking has increased. She has also suffered a relapse of her Crohns, with an average of 10 bowel movements each day,
persistent bloody diarrhoea, abdominal cramps and weight loss. She also noticed that when she carried out her usual home monitoring of her blood pressure and diabetes, both were out of control. On examination by her GP,
Lavinia was anxious, pale with slight tenderness in her right iliac fossa. Rectal examination revealed no abnormalities. The GP discussed the use of prednisolone to manage Lavinias clinical symptoms but she explained that
during the last flare-up of her condition 10 years ago, she was unable to tolerate them as she constantly felt agitated and restless and her blood pressure and diabetes were not well controlled. In the light of her intolerance of conventional steroids, the GP prescribed another drug.
