Case managers must devise methods for communicating the requirements of their clients to multiple agencies. Case managers can help their clients by advocating on their behalf. This entails portraying their clients’ circumstances in a compelling enough manner to entice multiple agencies to become involved and actively participate. For this weeks Discussion, you explore methods in which case managers might facilitate collaboration among several agencies.
To get ready for this discussion, do the following:
-Read through Chapter 5 of Case Management from an Empowerment Perspective: A Guide for Health and Human Service Professionals. Consider the notions about community and teamwork.
-Read and discuss the article “Case Management and Service Integration: How Service Delivery Systems Shape Case Management.” Concentrate on the case management procedures described.
-Read the article Georgia’s Case in the media.
-As a case manager, think about Georgia’s important needs and what recommendations you might offer. Consider how you could develop Georgia’s service strategy by coordinating services.
-By Day 4, post a summary of Georgia’s urgent needs. Then make three recommendations for services that will address those needs. Finally, describe how you would coordinate those services in order to establish an effective Georgia service plan.
