Description
Chapter 4a: Plan and Cost-Benefit Analysis.
· Pre-writing Activity:
· The research evidence and state of the science about what has or has not been effective to achieve the desired future state and b) the internal evidence from the organizational assessment that identifies what is feasible, meaningful, actionable and realistic from the organization’s perspective.
· Note: If you and your team consider various alternatives, but reject them based on feasibility, time constraints or cost, make note of this and include this at the end of this section of your paper.
· Writing: describe this plan in the first section of this chapter. The student needs to be clear about what the evidence-based improvement will actually entail , and which components of the fishbone diagram and/or process flow diagram will be improved as the result of the proposed change.
· Writing: write a cost-benefit analysis that identifies the cost of continuing with the current state related to the problem compared to the cost of the desired future state. What is the cost of all components of the proposed intervention?
o
may consider the costs of human resources (paid time for administrative support, clinicians, staff, others involved in the implementation/ innovation), startup costs (photocopies, charts, posters, educational materials, etc.), capital costs (equipment, supplies), operational costs (other ongoing costs related to the implementation), potential revenue or savings related to the future state.
If applicable, explain how the benefits outweigh the costs. Use graphic illustrations or tables to make selected points.
The student should also include the non-financial costs and benefits (e.g. patient satisfaction, quality of life or improved clinical outcomes )
Explain alternative options and provide a rationale for why certain ideas were included or excluded based on evidence, feasibility, cost, etc.Chapter 4b: Theoretical Framework for Change
§ Provide a description of the change theory selected and explain the rational for its use in this situation and setting. Address expected sustainability of the process change.
§ Identify any potential barriers to change. This may include the history of the present organizational culture and climate, as well as project feasibility and specific requirements that the proposal will entail. Describe the plan to address these barriers, and how the plan will incorporate various team members to support the change.
§ Address expected sustainability of the process change.
Chapter 4c: Plan for Implementation
§ Provide a detailed description of the proposed improvement; how will the model for implementation be used to actually implement the plan (e.g. cycles of Plan-Do-Study-Act (PDSA)
§ Include a description of the expected role of the student and other stakeholder/team roles in the change process.
§ If staff education is part of the plan, prepare a content outline. The content outline will be an addendum to this proposal. Provide a rationale for education materials planned for use. State where the materials are from, with an appropriate reference. Include a letter of permission for use or documents if the information was freely available. If there is a charge, describe how the unit, department, facility plans to purchase. If the materials will be made by student, explain why and also explains how such materials will be sustainable for use. Also, describe who is expected to attend the education sessions, how often the sessions will be offered to reach the intended expected audience, and who will teach the sessions.
Chapter 4d: Conclusion to the DNP Project Proposal
Write a concluding paragraph summarizing the content of the proposal (e.g. “This proposal has identified a clinically meaningful quality gap related to… analysis of the health system and care process demonstrated…and a review of the research evidence showed…)
Provide a transition from the proposal and plan to the next chapters which will describe the use of leadership approaches to engage the team in actual implementation of the project and process changeQuality Improvement Project in Health Care
