Introduction

Synopsis of Incident
Synopsis of Incident relays all important information that lead to the incident. This is the storytelling portion of the report. Summarize the incident. Causation
Causation needing to be addressed. Address the probable cause of the incident. Explain what was stated within the case study that caused the incident. Decision Criteria
Decision criteria addressed and correlated to application of course content. What was the decision/result/conclusion of the incident? What was determined to be the causation of the incident? Where there other similar incidents that occurred to assist in the confirmation of the causation. Provide sound reasoning the led to the decision criteria? Analysis
Analysis of the causation/observations/findings. Relate back to set decision criteria as well as topics learned through research and course content (safety bulletins, regulations, etc…). Based on analysis of causation, where there other incidents within the industry that could have been prevented? Expound upon specific weather theory that caused the incident. Analyze content within the course and correlate with incident, causation, and decision criteria. The majority of your research should be included within this section. Implications
Address the future implications should the problem of causation not be addressed. Express the “why” the need for the investigation of the researched incident. Recommendations
Recommendation for future implications within the industry. Recommendations should relate back to the incident discussed. They should be stated clearly and expressed as action statements or directives to do something. Express the insights gather from the causation to prevent future incidents. Be sure to use persuasive or firm language such as ‘must,’ ‘ought to,’ etc… Coordinate your research to this section. Do not just state personal recommendation. All recommendation should be researched and verified. Be sure cite where and how you contrived any recommendation. This should be, at minimum, 1 page written in APA formatting. Conclusion
Conclusion restates the focus of the report, sums up all points, and links to recommendations and personal narrative. The conclusion should be focused on summarizing the issue and the importance of additional research on the topic. The conclusion should restate the focus of the report.
