Jim Taylor is a 68-year-old man with motor neurone disease, diagnosed eight years ago. Prior to his diagnosis, Jim smoked 10 cigarettes a day from the age of 16. Jim had previously been diagnosed with chronic obstructive pulmonary disease (COPD) at the age of 54. Although Jim has significantly reduced his smoking to four cigarettes a day, he continues to smoke. Recently Jims mobility has rapidly deteriorated and he has become unable to walk. He also experiences limited movement of his upper limbs. Jims speech is deteriorating and he is finding it increasingly difficult to swallow and communicate. As a result, he is unable to swallow his saliva and is constantly drooling, which he finds distressing. Jim feels embarrassed to go out (due to the drooling) and no longer attends the games club he used to go to daily. His interaction with friends is therefore limited. Jim is fully conscious and has had a percutaneous endoscopic gastrostomy tube inserted for assisted nutrition. Jims prognosis may be measured in months. Jims General Practitioner has asked for prescribing advice on managing the symptom of drooling.
