Clinical Research Paper on Multiple Sclerosis Case Study
Important Guidelines for the Assignment
You’ll next create a differential diagnosis list, create a treatment plan, and submit a written clinic report detailing your patient’s care. Four diagnoses should be on your differential diagnostic list, one of which should be your final diagnosis.
Please provide a brief explanation of why you would include or exclude a diagnosis from your working diagnosis list. For your rationale, provide 6 research nursing and medical references.
An opening statement that indicates what the paper will entail should always be included in the document.
Remember to include a cover page and reference using the most recent APA formatting change.
For more information on how this work will be graded, see the SOAP NOTE TEMPLATE.
You must present a 12-15 page well-written evidence-based research paper that follows the rubric criteria, uses APA format for references, and includes at least 5 to 7 nursing or medical evidence-based research papers that are not older than 7 years.
Please read and evaluate it carefully; if the paper does not meet the standards of a master’s degree, I will not accept it. Before I allocate it to anyone, I just want to be clear about it. This is an advanced degree job that necessitates a strong writer with a background and experience in the nursing and medical fields, as well as a solid command of the English language and written communication.
Case Study B: A Patient’s Headache
The patient is a 42-year-old woman who suffers from urine incontinence. She claims to have urinated twice in the last 24 hours by mistake. She also stated that she has been feeling particularly tired and fatigued for the past six months. She went on to say that her lower extremities had been weak and fatigued for the past three months, with no apparent cause. The patient has also had periodic hazy vision for the past three months. During urinating, the patient denied any pain,
burning feeling, or vaginal odor. Patient has a history of an automobile accident that left her with numbness and tingling in her right leg ten years ago, although she claims she hasn’t had any paresthesia in years. As previously noted, she has uneven gait and full lack of control in her lower extremities, which can continue for a few seconds. She claims that resting at night does not make the exhaustion go away. Clinical examination and physical findings reveal bilateral lower
extremity weakness, which is linked to a neurological illness or difficulties with the central nervous system.
The Main Complaint:
Reports from patients “Yesterday, I experienced two mishaps. I peed on myself by accident. It makes me feel extremely humiliated.” In addition to the patient Problem statement, she mentions feeling especially weary in the last few months (6 months to be exact).
History’s Reliability and Source:
The patient is attentive and oriented, and he or she is able to respond to the majority of the questions.
Data that is both subjective and objective.
Hint 1: Multiple sclerosis is the patient’s principal diagnosis; other possible diagnoses include:
Infection of the Urinary Tract
Anemia is a condition in which a person’s blood is
Vitamin B deficiency is a condition where you don’t get enough of a certain vitamin.

Hint 2 (a patient’s treatment plan that includes the most up-to-date Multiple Sclerosis treatment)
3rd hint
All of the following should be included in the patient lab, and the rationale for these labs should be explained with references to back it up.
Urinalysis is a test that examines the contents of the urine.
The brain and spine are scanned using magnetic resonance imaging (MRI).
CBC in combination with BMP (Especially to test for vitamin B12 deficiency)
EMG is an acronym for Electronic Music Group (electromyogram)
Cerebrospinal fluid analysis is a procedure that examines the fluid in the brain and spinal cord (CSF)
ANALYSIS (Antinuclear Antibody Test)
CSF is a type of cellular symbios (Cerebrospinal fluid analysis)

SOAP Notes Typing Template:
Assessment
This is where you’ll find the section about differential diagnoses. Make a list of FOUR possible diagnosis. Your final diagnosis will be one of these. List a brief justification for EACH of your diagnoses, suggesting why this diagnosis should be considered or not as the final diagnosis. Include any references you utilized to help you think.

2. Infection of the Urinary Tract
Anemia is number three.
4. Deficiency in vitamin B
5. Amyotrophic Lateral Sclerosis (Amyotrophic Lateral sclerosis)
Plan
The following aspects are present in all plans till you graduate. Please note whether an item is not appropriate or needed at this time if it does not apply to the specific situation. It’s assumed that if it’s not mentioned, it wasn’t discussed.
Prescription and over-the-counter medications are kept in this section of the pharmacy. Prescription medications should be written up just like a prescription.
Non-Pharmacology What non-pharmaceutical interventions would you suggest for your patient?
Diagnostics Would you wish to order any lab tests, radiography, or other diagnostics? Always keep primary care in mind. In the office, there are no troponins or bedside stat echos.
Consultations / Recommendations Do you want to call a friend? Should they seek increased or further care from a specialist?
Patient Orientation What critical information do you need them to be aware of? Don’t overlook the possibility of significant pharmaceutical interactions or OCP.
Continuation Consider how much wiggle room you want to offer this patient before anyone sees them again. Is it really only one week? Two? What should they do if their symptoms change or get worse?
