Clarity of Knowledge Researched of the Disease State
(20 pts)
Flow, grammar, ease of reading, documentation of data presented in a format for the reader to understand the disease process based on demonstration of professional writing with usage of own expressive writing . Reported data is clearly presented and is captured in the correct template descriptor thatstrongly supports researched disease state. No or minimal grammatical errors, etc. (16-20pts)
Flow, grammar, ease of reading, documentation of data presented in a format may be limited for the reader to understand the disease process based on demonstration of professional writing with usage of own expressive writing . Reported data is presented and captured in the correct template descriptor that mildly supports researched disease state. Few grammatical errors, etc. (11-15pts)
Flow, grammar, ease of reading, documentation of data presented in a format may be limited for the reader to understand the disease process based on demonstration of professional writing with usage of own expressive writing . Reported data is presented and captured/ documented under two incorrect template descriptors that will limit the supported researched disease state. Few or more grammatical errors, etc.
(7-10pts)
Flow, grammar, ease of reading, documentation of data presented in a format is moderately limited for the reader to understand the disease process based on demonstration of professional writing with usage of own expressive writing . Reported data is presented and captured/ documented under two incorrect template descriptors that will limit the supported researched disease state. Few or more grammatical errors, etc.
(3-6pts)
Data presented has made the flow and ease of reading difficult for the reader regardless of the template followed and with usage of expressive or limited expressive writing. Few or more grammatical errors, etc. exist to determine the clarity of knowledge present. (0-2pts)
Accuracy of Research Relating to Disease State Presented
(20 pts)
Pathology of disease state clearly defined, explained; research is proven, accurate in reporting information throughout the report with journal article data interwoven to strongly support valid and concise points of the severity of the disease state. Flow, grammar and succinctness are demonstrated in accuracy of reporting data with the ability of the reader to follow the progression of the disease state to treatment outcomes. (16-20pts)
Pathology of disease state slightly defined and explained; defined, explained; research is proven, mostly accurate in reporting information throughout the report with journal article data interwoven to slightly support valid and concise points of the severity of the disease state. Flow, grammar and succinctness are demonstrated in accuracy of reporting data with the ability of the reader to follow the progression of the disease state to treatment outcomes. (11-15pts)
Pathology of disease state slightly defined & explained; research is somewhat proven & is mostly accurate in reporting information throughout the report with journal article interwoven that limits valid and concise points of the severity of the disease state. Flow, grammar and succinctness are slightly demonstrated in accuracy of reporting data with the ability of the reader to follow the progression of the disease state to treatment outcomes with mild degree of difficulty. (7-10pts)

Flow, grammar and succinctness are slightly demonstrated in accuracy of reporting data with the ability of the reader to follow the progression of the disease state to treatment outcomes with moderate degree of difficulty. (3-6pts)
Pathology of disease state slightly defined & explained; research is somewhat proven & is lacking accuracy in reporting information throughout the report. Journal article may be interwoven that limits or does not make relevant valid and concise points of the severity of the disease state. Flow, grammar and succinctness are lack in demonstrating accuracy of reporting data with the ability of the reader to follow the progression of the disease state to treatment outcomes with moderate degree of difficulty.
(0-2pts)
Descriptive Nature of Reasearch(10pts)
Demonstrates a high level of expressive and professional writing to strongly describe the prominence of the severity of the disease state by combining critical thinking skills in drawing two conclusions from the research literature, illustrations correlated with descriptive text in respective template categories. Demonstrates an accurate interpretation of the progression of the etiology to final treatment outcomes.(9-10pts)
Demonstrates a moderately- high level of expressive and professional writing to strongly describe the prominence of the severity of the disease state by combining critical thinking skills in drawing one-two conclusions from the research literature, illustrations correlated with descriptive text in respective template categories. Demonstrates an accurate interpretation of the progression of the etiology to final treatment outcomes.(7-8pts)
Demonstrates a moderately- low level of expressive and professional writing to mildly describe the prominence of the severity of the disease state by combining critical thinking skills in drawing one conclusion from the research literature, illustrations correlated with descriptive text in respective template categories. Demonstrates an accurate interpretation of the progression of the etiology to final treatment outcomes.(5-6pts)
Demonstrates a minimal level of expressive and professional writing to mildly describe the prominence of the severity of the disease state by combining critical thinking skills in drawing one conclusion from the research literature, illustrations correlated with descriptive text in respective template categories. Demonstrates an accurate interpretation of the progression of the etiology to final treatment outcomes.(4-3pts)
Lacks demonstration of expressive and professional writing to describe the prominence of the severity of the disease state by combining critical thinking skills in drawing none-one conclusion from the research literature, illustrations correlated with descriptive text in respective template categories. Difficult to correlate based on writing presentation with no clear delineation of data presented. Demonstrates a lack of accurate interpretation of the progression of the etiology to final treatment outcomes. (0-2pts)

A newborn infant has been evaluated by the pediatric intensivist and has been referred to the on-call pediatric cardiologist. At birth, the baby girl, presents with a cyanotic heart disease, with a loud systolic ejection murmur heard best at the upper left sternal border. Additional ausculatory findings are described as a systolic murmur with a thrill best heard at the lower left sternal border with a single S2 (A2 only). The baby has hypoxic spells with any movement (Dyspnea on exertion). An echocardiogram with color flow and continuous wave Doppler is ordered STAT. The echo/Doppler interpretation is Tetrology of Fallot with the four classic cardiac abnormalities. Chest X-ray reveals a boot-shaped heart due to the right ventricular hypertrophy with right ventricular and right atrial enlargement. Electrocardiogram findings demonstrate right axial deviation and confirm the right sided abnormalities seen on the chest x-ray.

Critical Core Assignment
Assignment: Congenital Disease ReportCT KI-3: Student evaluates concepts and information to support an opinion, solve a problem or reach a conclusion
Objective: Demonstrate the ability to define, understand, and evaluate a congenital heart disease from embryological development to treatment options.
Instructions:
I. Disease Report Requirements and Paper Format Requirements
Instructor will provide a list of congenital heart diseases to be chosen by the student
Cover sheet with Disease State Listed, Student Name, Course, and Date. Minimum of 7 pages long.
In-depth research is expected from scientific journals, course textbooks and class discussions
ADA compliant with Arial, Calibri, and Verdana text style, 1.5 line spacing and a 10-12 font
Medical illustrations and images are to be embedded throughout the report to prove descriptive writing. Images are not to fill a full page, if so then this will not count as the minimal pages met. Illustrations/pictures are to be downloaded directly from cited researched material, or journal article to make correlation to your descriptive discussion of your report.
Writing style needs to convey a consistent professional tone making it clear and easy to read. Using a professional writing style will enable you to elaborate about your disease state.
Under the headings on the Congenital Disease Report Template. Research the disease state and the findings that correlate with heading and specific disease. In-depth descriptive writing is required to demonstrate a cumulative knowledge of all aspects of cardiovascular principles.
Do not abbreviate chambers or structures in the Disease Report.
