Mr Genadaire is a 56-year-old man with known severe left ventricular systolic impairment secondary to ischaemic heart disease. His GP has referred him to the cardiology clinic, with extensive peripheral oedema and diagnosed as decompensated heart failure with a NYHA score of III/IV. His past medical history also includes diabetes type 2 treated with insulin, diabetic peripheral neuropathy and a false joint in his sternum.
On examination, Mr Genadaire was generally well, with no shortness of breath, orthopnoea, chest pain, palpitations or syncope. His exercise tolerance was only limited by cramps in his legs. His vital signs were within the normal range: temperature 36.8ºC, HR 76bpm, BP sitting 110/55mmHg and standing 112/55mmHg. His weight was 85.7kg, which was a significant increase compared with his last weight of 71kg two months ago.
His current medication included bumetanide 1mg twice daily, spironolactone 25mg once daily, aspirin 75mg once daily, metformin 1g twice daily, NovoMix® 30 (biphasic insulin aspart), ramipril 5mg twice daily, simvastatin 40mg once daily, bisoprolol 3.75mg once daily and bendroflumethiazide 5mg twice weekly.
The primary aim of the heart failure team was to try to resolve the extensive oedema and optimise his heart failure treatment.
