SAVE-ME Hospital ED and Forecasting Data
Of the many challenges confronting hospital-based emergency and trauma care today, one of the most serious is that of capacity. As reported in the IOM Committee’s Hospital-Based Emergency Care:
Between 1993 and 2003, ED visits increased from 90.3 to 113.9 million, a 26 percent increase. During this same period, the United States experienced a net loss of 425 hospital EDs…. The result of this growing imbalance between demand and capacity is a nationwide epidemic of ED overcrowding, boarding, and ambulance diversion (p. 19).
For this Discussion, you will assess how forecasting and capacity management can be used to help prepare an ED to meet the demand for its services.
To prepare for this Discussion:
Read the scenario and questions for Exercise 14.7 in Quantitative Methods in Health Care, page 392.
Download the Forecast for SAVE-ME Hospital ED spreadsheet, which provides the numerical solutions to Exercise 14.7.
Take some time to examine and interpret this spreadsheet. Note that it provides a forecast for emergency and non-emergency cases arriving at this ED for 6 months in the future. Identify results that are significant in terms of capacity management issues.
Review the Hospital-Based Emergency Care excerpt (pages 1–4), from this week’s required resources, regarding key challenges in EDs. Refer also to the forecasted data provided in the 14.7 solution spreadsheet. Based on this information, and doing further research as needed, identify some operational issues that would need to be addressed so that this ED is able to meet capacity demands in 6 months.
Post by Day 4 a response to the following:
Select and interpret some significant results in this spreadsheet. What do the numbers you selected mean in operational terms?
What are one or two specific operational issues that the manager of this ED would need to address in light of these 6-month forecasts, either for the emergency or non-emergency cases? Discuss the implications for the manager and staff of these anticipated changes in capacity requirement.
Read a selection of your colleagues’ postings on both scenarios, selecting one posting to respond to from either this or the other Discussion thread.
Respond by Day 6 to at least one of your colleagues’ postings on either of the Discussion threads in one or more of the following ways:
Ask a probing question.
Expand on the colleague’s posting with additional insight and resources.
Offer polite disagreement or critique, supported with evidence.
In addition to, but not in place of the above, you may:
Offer and support an opinion.
Validate an idea with your own experience.
Make a suggestion or comment that guides or facilitates the Discussion.
Return to this Discussion in a few days to read the responses to your initial posting. Note what you have learned and/or any insights you have gained as a result of the comments your colleagues made. No posting is required.
Memorial Hospital Emergency Department
Of the many challenges confronting hospital-based emergency and trauma care today, one of the most serious is that capacity.
To prepare for this Discussion:
Read the scenario about Memorial Hospital Emergency Department on pages 17–18 in the IOM Committee’s Hospital-Based Emergency Care: At the Breaking Point, http://books.nap.edu/openbook.php?record_id=11621&page=17
Identify specific capacity management issues facing this ED, such as bottlenecks, and possible causes for these.
Review Chapter 5 in Health Care Operations Management, and Chapter 2 in Quantitative Methods in Health Care Management to identify forecasting methods that might be appropriate in the ED. What can and should be forecast in an ED? What kinds of quantitative forecasts and other capacity planning efforts might have helped Memorial Hospital’s ED avoid the problems it faced in this scenario?
Post by Day 4 a response to the following:
Describe 2–3 capacity management issues facing the ED in this scenario, and identify likely causes for them.
Discuss two specific kinds of forecasts or other methods involving capacity planning that managers at Memorial Hospital’s ED might have conducted to help avert the problems it faced in this scenario. What are possible implications for managers and staff from these kinds of forecasts or capacity planning?
Read a selection of your colleagues’ postings on both scenarios, selecting one posting to respond to from either this or the other Discussion thread.
Respond by Day 6 to at least one of your colleagues’ postings on either of the Discussion threads in one or more of the following ways:
Ask a probing question.
Expand on the colleague’s posting with additional insight and resources.
Offer polite disagreement or critique, supported with evidence.
In addition to, but not in place of the above, you may:
Offer and support an opinion.

Return to this Discussion in a few days to read the responses to your initial posting. Note what you have learned and/or any insights you have gained as a result of the comments your colleagues made. No posting is required.
Goals and Approaches of Health Care Operations Management
To make the best use of the concepts and tools of operations analysis, a manager must understand what health care operations management actually encompasses.
For this Discussion, you are asked to summarize the functions and goals of operations management and explain how quantitative analysis can further operationally aims in a selected health care setting.
To prepare for this Discussion:
Review the various descriptions in this week’s Learning Resources about the aims and functions of operations analysis and management. Pay particular attention to Table 1-1 on page 7 (“Key Functions and Issues in Healthcare Operations Management”) in the Langabeer text and Table 1-3 on page 22 (“Roles and Trends in Healthcare Operations Management”), as well as to the descriptions in Chapter 2 of hospital business operations (pages 41–43). How would you summarize the purpose of operations management?
Make a note of insights that you gain from this week’s media segments on the role of operations management within health care organizations.
Review pages 17–19 in the Langabeer text noting in particular the scientific and mathematical concepts that serve as the foundation of the operations management discipline. Consider the ways in which quantitative analysis or other scientific approaches would make a difference in operations management.
Select one type of health care setting of interest to you (e.g., community hospital, academic hospital, nursing home, home care agency, assisted living, ambulatory surgery/diagnostic center, and so on). Identify ways that data or quantitative analysis is being used, or could be used, to address significant operational issues in this setting.
Note: If you need help identifying how quantitative analysis is contributing in your selected health care settings, do a search through the Walden Library. Review recent issues of the journal Healthcare Management Science or other journals and trade magazines that focus on your selected health care setting. Read through abstracts to get a sense of operational issues that are being addressed through quantitative approaches. You are also encouraged to review the supplemental readings listed this week from previous required textbooks in MHA 6115 and MHA 6160, with an eye to these applications of quantitative methods.
Post by Day 4 a response to the following:
Briefly describe the scope and purpose of operations management within health care organizations.
Summarize a significant insight that you gained from this week’s media segments regarding challenges or objectives of operations management.
Identify the health care setting you selected. Explain one or two specific ways that quantitative analysis is contributing, or could contribute, to the aims of operations management in your selected health care setting.
Read a selection of your colleagues’ postings.
Respond by Day 6 to at least one of your colleagues’ postings in one or more of the following ways:
Ask a probing question.
Expand on the colleague’s posting with additional insight and resources.
Offer polite disagreement or critique, supported with evidence.
In addition to, but not in place of the above, you may:
Offer and support an opinion.
Validate an idea with your own experience.
Make a suggestion or comment that guides or facilitates the Discussion.
Return to this Discussion in a few days to read the responses to your initial posting. Note what you have learned an
DISCUSSION 1:
Factors that Cause Crime
In Chapter 1 of Crime Prevention, the author discusses factors that cause crime. Select a specific crime or criminal issue. What are three factors that lead to the crime or criminal justice issue you identified? Identify possible solutions to the crime or criminal justice issue you selected. Why do you think these solutions would be effective solutions? Are your solutions cost effective?
Your initial post must be a minimum of 250 words in length. Support your post with examples from your required reading material and /or other scholarly sources, and provide in-text citation in APA style.
