Mr David Jones is a 67-year-old retired bus driver who was referred by his GP to the rapid access chest pain clinic (RACPC) with a three-month history of chest pain on exertion, particularly when walking up an incline. He has nochest pain when resting. Mr Jones also has type 2 diabetes, is hypertensive with raised cholesterol levels (total 5.5 mmols/l, HDL
1.0 and LDL 3.4) and is allergic to aspirin. He states that the first time he took aspirin his face swelled. His current medications are metformin 500mg twice a day; enalapril 20mg once daily and simvastatin 40mg nocte.
On assessment in the RACPC, Mr Jones is not distressed or experiencing any pain. His vital signs are temperature: 36.8ºC, pulse rate 80bpm, regular; ECG normal sinus rhythm and BP 145/85mmHg. Based on Mr Jones clinicalhistory and presenting symptoms, he was rated as being at high (97%) risk of having coronary artery disease (NICE 2010). He was commenced on medical therapy for his presenting symptoms and recommended for a coronary angiogram as well as a number of investigations including a full blood count, urea and electrolytes, blood glucose and liver function tests.
