The Scenario
Take on the role of the informaticist in a large healthcare organization. One of the areas you are responsible for is a busy, 30-provider medical office. The organization (thus the clinic) is an accountable care organization (ACO), and you are
responsible for ensuring all the documentation tools needed to capture the quality data are in place and functioning as expected. The site manager has come to you with some concerns regarding the provider efficiency (PE) report.
The PE report provides stakeholders a clear view of progress toward meeting various metricsincluding the ACO quality measures. The report can show data from an organizational level or can be filtered down to show clinic progress or even
individual provider progress. The site manager is looking at data for the entire clinic and is specifically concerned about the quality measure for falls. This measure focuses on screening patients for future fall risk.
You previously worked with your team to create a fall risk assessment tool to capture the fall risk data in a discreet format so that it is easily captured and seen on the PE report. Based on the report, the office is not meeting the regulatory
requirement which puts the practice at risk of not being compliant with the quality measure and losing ACO dollars. An additional concern is the safety of patients if they are not being screened for a fall risk. It is not clear if the issue is provider driven or the report is not pulling all the correct data.
She has asked you to investigate and identify issues, causes of the issues, and recommend a solution to her and the other stakeholders.
