Mr Burton is a 59-year-old part-time lecturer in Ancient History who has suffered moderate heart failure with left ventricular systolic dysfunction for the past two years. His long-term condition has been well controlled on ramipril 5mg daily, bisoprolol 7.5mg daily, aspirin 75mg daily, and metolazone 2.5mg weekly. He enjoys his job but recently (especially during assessment periods) he has found it stressful and has been experiencing bouts of heartburn, for which he takes magnesium trisilicate liquid bought over the counter. The magnesium trisilicate is very soothing and he has been taking it regularly (up to 20ml four times daily after meals), as he thinks its a fairly harmless substance. He has visited his GP because he has noticed that he has become breathless, with swelling of his ankles. He is also feeling constantly tired and finds himself belching frequently
