When you are caring for a person with a reproductive disorder, you want to ensure you are upfront and transparent with them. With a patient with cancer that can affect fertility, you want to ensure you give them options to save their eggs or a sperm bank. It is essential to recognize that they might want children someday, so providing the option for it is key.

When caring for a client with urinary incontinence, it is essential to know what kind of incontinence is happening. If you know the type, the nurse might identify what is happening in the body and what can help relieve it. The nurse also wants to monitor their psychosocial due to this being very insecure. The nurse wants to ensure the patients know what to do and help eliminate incontinence. One of the big exercises is the Kegel exercises that they can do. They can also have a bladder training program to help get the urge to go again and retrain the bladder.

When caring for these patients, you want to ensure that you are maintaining the proper airway. This is the key to managing all respiratory infections and disorders. If a patient has stridor, it is a key finding that their airway is closing and that we, as nurses, need to ensure the proper airway is getting replaced.

When caring for these patients, the nurse wants to ensure we are implementing all exercises to try and get anything that is not supposed to be in the lower lobes of the lungs out of there. Some of the techniques the nurse wants to ensure they are implementing are cough and deep breathing, incentive spirometry, and ambulating whenever possible. This will help get all the gunk out of the lungs that are not supposed to be there. We also want to ensure we maintain a proper airway for the client so we can get everything out of the lung that we need to. In some cases, a chest tube might help get the excess fluid out of the lung.

When nurses are caring for a patient with cardiovascular disorders, they want to ensure that the blood pressure and heart rate are within normal range. If there is an abnormality in either of these, it can impact how the heart functions. If a patient has high blood pressure, the heart will work harder to compensate for the increased pressure. The same goes for if the blood pressure is too low. A fluid bolus might be given if the blood pressure is too low. If the pressure is too high, relaxation techniques or medication might be administered to help lower the blood pressure and heart rate. The main priority for a patient with cardiovascular disorders is ensuring adequate blood flow to the rest of the body and trying to minimize the strain of the heart. Some of these conditions make the patients eligible for a heart transplant. We, as nurses, want to ensure the patient stays stable enough to get a heart transplant.

When caring for a patient with a hematological disorder, it is important that we, as nurses, ensure we are providing the best care we possibly can. When caring for a patient in a sickle cell crisis, it is important to get their pain under control. This crisis can be very painful, so including the doctor and fellow nursing staff to ensure the proper pain regimen is being done for the patient. It is important to work with the patient to ensure they feel that they are getting the appropriate care that is needed. The nurse would need to provide that feedback to the providers. Everyone in the nursing staff will play a huge role in ensuring the proper care for a patient with a hematological disorder.

- This student was able to do her assignment for module 1 on the reproductive system disorder of uterine leiomyoma. Uterine leiomyoma are non-cancerous, slow-growing solid tumors of the uterine muscle layer that develop from excessive local growth of smooth muscle cells. This student was interested in this topic because I had not heard of this issue before. Nursing management for this patient would include pain management fluid replacement, bleeding control, and patient education.
- Determine appropriate multidimensional nursing interventions for clients with disorders of the urinary system.
- This student learned that disorders involving the urinary system can affect the entire body and the homeostasis of the entire body. The urinary system controls the fluid and electrolytes moving out of the body. Any issues with the urinary system would require the patient to increase their oral fluid intake. This will help to flush out any remaining electrolytes or toxins in the body. Sometimes nursing care for these patients will include catheter care and education regarding a catheter, which may be inserted for urinary retention.

- The upper respiratory system is made up of the nose, nasal cavity, mouth, pharynx, and larynx. This nursing student learned that the upper respiratory and lower respiratory involved very different things. Infection of the upper respiratory tract is very common and affects how oxygen moves in and out of the body. If the nose or mouth is blocked for some reason, oxygen has a harder time getting in and ventilation becomes a problem. Using our ABCs of nursing interventions, airway and breathing are the first 2 to pay attention to, and include upper respiratory disorders.

- During module 5, this student was able to do her assignment on cystic fibrosis. Cystic fibrosis is a genetic disease resulting from poor chloride transport and the formation of thick mucus that causes problems in the lungs, pancreas, liver, salivary glands, and the testes. This mucus narrows the airways and reduces the airflow getting to the patient. This nursing student learned so much about cystic fibrosis including the fact that it doesn’t just affect the lungs, but the gut of these patients as well. Cystic fibrosis patients are unable to absorb nutrients due to the secretions and are more susceptible to being dehydrated or malnourished. Nursing care for these patients need to be all-encompassing, including oxygen management, oral care, dietary management, and possible lung transplant.

- This student nurse chose to do her assignment for this module on acute cardiac tamponade. This occurs when excess fluid or blood collects in the pericardium surrounding the heart and inhibits the heart from fully contracting, which increases the cardiac pressure. This compromises the circulation of blood throughout the entire body and is considered a medical emergency. Nursing interventions for these patients include establishing venous access for fluid repletion and possible vasopressor therapy, frequent monitoring of vital signs, continuous cardiac monitoring, and promoting complete bed rest with the patient’s lower extremities elevated.

- This student nurse has some real-life experience with patients living with sickle cell disease and it was interesting to learn more about this process. Sickle cell anemia is a severe hemolytic anemia that results from inheritances of the sickle hemoglobin gene. Patients with this disease can present with anemia, jaundice, dysrhythmias, which can lead to infection, stroke, renal failure, or heart failure. Pain management and oxygen management are some of the major nursing interventions for these patients, but addressing the factors causing the crisis would be important. Pain is usually subjective but it is important to remember that these patients are in a lot of pain and to treat them as such. Education surrounding relaxation techniques, breathing exercises, and how to prevent a sickle cell crisis from happening.
