Mrs. J. is a 63-year-old married woman who has had hypertension, chronic heart failure, and chronic obstructive pulmonary disease in the past (COPD). Despite the fact that she requires 2L of oxygen/nasal cannula during activity at home, she continues to smoke two packs of cigarettes every day and has done so for 40 years. She developed flu-like symptoms three days ago, including fever, active cough, nausea, and lethargy. She has been unable to execute ADLs for the past three days and has needed assistance walking small distances. For the past three days, she has not taken her antihypertensive or heart failure meds. She was taken to the hospital’s intensive care unit today with acute decompensated heart failure and COPD exacerbation.
Subjective Data
Is quite worried and wonders if she is going to die.
She denies any pain, but claims she can’t get enough breath.
Her heart feels like it’s “going away,” she says.
She claims to be fatigued and unable to eat or drink on her own.
Data that is objective
175 cm tall, 95.5 kg heavy.
T 37.6 degrees Celsius, HR 118 and irregular, RR 34, and BP 90/58.
Cardiovascular: Distant S1, S2, S3; PMI at sixth ICS and faint: all peripheral pulses are 1+; bilateral jugular vein distention; initial cardiac monitoring shows a ventricular rate of 132 and atrial fibrillation.
Pulmonary crackles; reduced breath sounds in the right lower lobe; coughing frothy blood-tinged sputum; SpO2 82 percent
Hepatomegaly 4cm below costal margin. Gastrointestinal: BS present.
Intervention
Her symptoms are controlled by the following drugs, which are delivered by pharmacological therapy:
intravenous furosemide (Lasix)
Enalapril is a drug that is used to treat high blood pressure (Vasotec)
Metoprolol is a drug that is used to treat high blood pressure (Lopressor)
morphine sulfate IV (Morphine)
Short-acting bronchodilator inhaled (ProAir HFA)
Corticosteroid inhaled (Flovent HFA)
At 2L/NC, oxygen is given.

Outline COPD triggers that can lead to more frequent exacerbations and repeat visits. Discuss what smoking cessation options should be offered to Mrs. J. in light of her present and long-term tobacco use.
