Charlie Dee, a 56-year-old male client, has smoked two packs of cigarettes per day for the past 30 years. After being diagnosed with chronic obstructive pulmonary disease, he is visiting nurse practitioner Sandra Gee for the first time. Eliciting the client’s narrative of his experience with this sickness, his impressions of his health, and how the disease manifests itself is the first step in pattern appraisal (symptoms). Mr. Dee claims that he has a productive cough that becomes worse in the mornings, that he is short of breath when he exercises, and that he is always weary. The nurse practitioner discovers that Mr. Dee’s sleep patterns and dietary consumption have changed after asking him particular questions. He sleeps for shorter periods of time and eats less. She also discovers that Mr. Dee’s wife smokes and that their pets are indoor cats. He doesn’t believe his wife will be willing to change her ways or get rid of the cats. During this assessment, the nurse tries to figure out what matters to Mr. Dee and how he defines health.
Mutual patterning entails exchanging information and providing options. The nurse recounts what she has been told and how she understands it at the end of the appraisal. In this approach, the nurse and the client may agree on what activities are appropriate for Mr. Dee. Ms. Gee informs him about the disease and makes solutions to help him feel better. Breathing retraining, advice for a high-protein, high-calorie diet, eating smaller meals more frequently, sleeping with the head raised, and performing progressive relaxation exercises before bedtime are all noninvasive therapies. The nurse advises the Dees to purchase a high-efficiency particulate air (HEPA) filter and humidifier to help remove pollutants from the air and maintain correct humidity levels in the home.
Because Mr. Gee has shown a wish to quit smoking, the nurse proposes that he complement his nicotine patches with forms of centering, such as guided imagery and meditation. She also gives him written information regarding the disease, which he can give to his wife. Mr. Dee says he feels better knowing he has the capacity to improve some aspects of his life at the end of the visit.
