Mr. Edwards is a 75-year-old man with a two-year history of chronic obstructive pulmonary disease. He continues to smoke 12 pack of cigarettes per day, has no exercise regimen, and must do self-care activities slowly due to exhaustion. He sees no reason to increase his fluid consumption. He is currently being treated for right upper lobe pneumonia and has a cough that produces thick, yellow sputum on a regular basis. When he is lying flat, he coughs more frequently. He is married, and his wife, Kathy, is by his side in the hospital.
Assessment
Mr Edwards’ SpO2 ranges from 78 to 84 percent, with an average of 84 percent this morning. T 101.4F, HR 102/min, RR 30/min, and BP 130/90mmHg are his other vital signs. He’s shivering and has experienced some diaphoresis. His ribs are hurting him, and his mouth is dry, he says. Mr Edward’s mucous membranes and skin are both dry upon examination. Crackles can be heard bilaterally in the lower lobes. His sputum is yellow to yellow green in color and thick. His respiratory condition will improve if he gradually increases his exercise, drinks more fluids, and stops smoking, according to his health care provider. In bed, he’s in a semi-fowlers position.
