Characteristics of Good Psychodynamic Case Formulations
What makes a case formulation psychodynamic?
A psychoanalytic or psychodynamic formulation is based on the following assumptions:
The relationship between conscious and unconscious meanings, feelings and motives is primary in understanding psychopathology.
Sexual, aggressive and dependency (attachment) feelings/impulses structure our relationships, making us fear the impact these may have on our relationships and our own and others welfare.
All psychological life involves conflict between competing motives, feelings, and impulses, which causes anxiety or psychic distress.
We protect ourselves from this anxiety/distress by means of unconscious defences.
All psychological symptoms and pathological personality traits are meaningful expressions of psychic conflicts and how we manage these.
Our personalities are largely shaped by the subjectively perceived emotional quality of early childhood interactions with primary caregivers and siblings, where our conflicts first arise, and which provide the templates for subsequent relationship difficulties.
The characteristics of a good psychodynamic formulation
It accounts for all or most of the known facts of the case, the influence of unconscious influences, including psychological defences, and does not omit any important case information.
It focuses on the specific details of the individuals particular symptoms, history, experience, and behaviour, and shows the links between the component aspects of the persons core conflicts instinctual wishes/hidden feelings, anxiety, and defences.
It accounts for how the persons history and personality predisposed him/her to develop these symptoms and how they are meaningful expressions of the persons core psychological conflicts.
It accounts for why the problem emerged when it did and why the problem persists in the present.
