On his final visit to the clinic two months later, Mr Genadaires peripheral oedema was resolved (weight reduced from 85.7kg on initial visit to 72.6kg) with the changes in his medication.
Indeed, he reported feeling well and all other parameters were within the normal range except for his renal function which was now impaired, with creatinine levels elevated from normal levels to 244 micromol/L (60110) and urea 35.7mg/dL (normal 620). Both sodium and potassium levels were within normal range. It was puzzling why his renal function should suddenly have deteriorated. On further questioning it emerged that he had returned from Africa a few days ago and suffered severe diarrhoea and vomiting which had now ceased. His current medication included bumetanide 1mg twice daily, spironolactone 25mg once daily, aspirin 75mg once daily, metformin 1g twice daily, NovoMix® 30 (biphasic insulin aspart), ramipril 5mg twice daily, simvastatin 40mg once daily, bisoprolol 3.75mg once daily and bendroflumethiazide 5mg twice weekly.
