Sally is a 21-year-old girl who came to see you after a suspected epileptic seizure that morning, as she was trying to wake up. According to her father, Sally has had other seizures over the last two years, all of which have been related to her sleep pattern. She has never previously been medicated, due to the spasmodic nature of the seizures and her difficulty in getting a witness account. After a consultation, Sally was referred for an electroencephalogram (EEG) with sedation. During the appointment, she had another seizure, which was witnessed. Sally was noted to have a hemiparesis post-seizure, with a drooping eyelid and drooping mouth, and was also
incontinent of urine. Sally was admitted and an EEG confirmed a diagnosis of temporal lobe epilepsy with generalised tonic-clonic seizures (complex). She was sent for an urgent scan, which showed an area of scar tissue in her
left temporal lobe, believed to be the result of a childhood accident. There are no signs of tumour, no fever and all biochemistry is normal. After one hour, the hemiparesis resolved. Sallys boyfriend was very worried that she might have a brain tumour or could have suffered a stroke but Sallys medics confirmed that they felt this was a residual Todds Palsy, which had corrected after full seizure recovery.
